Please…
- fill online forms (i.e. Medical History Form, Health Screening Form) prior to your appointment. You will receive email from containing the online form from us.
- attend the surgery alone wherever possible or with one other person if absolutely necessary. (i.e. children under 16 years old, carers)- We have limited capacity in the waiting room.
On your Arrival

- wear a facemask inside the building (unless you are exempt)
- use the handgel
- maintain the social distance -please wait outside if someone is coming downstairs
